Have you been off work for seven days or fewer?

You do not normally require a fit note from the practice if you have been unable to work for 7
consecutive calendar days or fewer.
The 7 days include:

  • Saturdays and Sundays
  • Bank or Public Holidays
  • Days when you would not normally work

You should complete a self-certification form and provide this directly to your employer.

You do not need to contact the practice.

Please note, if your employer asks for medical evidence covering the first 7 days of your absence,
this is not normally provided as an NHS service and a fee may apply

    Upon submission of your request, please note the following:
    • Your request will take 2 working days to be reviewed by a GP.

    • Where the GP is unable to issue the sick note, the practice will contact you to discuss further

    • If you have not heard back from the practice after 3 working days, you can assume that your sick note can be collected from our main reception desk

    • We do not email, post or fax sick notes. These must be collected from our main reception desk.
